NOTE: One registration form may be used for all quilt entries; limit
six per exhibitor. You also need to print, fill out and attach an exhibit
tag to each quilt or wall hanging you will be entering. How many total quilts are you entering in the exhibit? ________ (limit 6 per exhibitor) Please resister each quilt in only one category below. If entering more than one quilt, use this single form to list the total number of quilts in each category you will be entering. In order to qualify for the judging competition, the quilt exhibitor must have had some part in the making of the quilt. Using a straight pin, attached the form to the front left edge, halfway up your quilt with “information for judges” facing up. For wall hangings, pin the form at the bottom left of the quilt.
